Please make sure to fill out the required form exactly as shown below and verify that all information entered is complete and accurate before submitting. Incorrect or missing details may delay your onboarding.

To ensure fast and accurate onboarding, all vendors must submit their information using the required Excel format below.

Required Columns

Your Excel file must include the following columns in this order:

Company Information

  • Company Name

  • DBA (optional)

  • Company Street Address

  • Company City

  • Company State

  • Company ZIP Code

  • Website (optional)

  • Company Email

  • Company Type

  • Tax Identification Number

Office Information

  • Office Location Street

  • Office Location City

  • Office Location State

  • Office Location ZIP Code

License Information

  • License Number

  • License State

  • License Class (optional)

  • License Expiration Date

Contacts

  • Primary Contact First Name

  • Primary Contact Last Name

  • Primary Contact Email

  • Default Signer First Name

  • Default Signer Last Name

  • Default Signer Email

  • Default Task Assignee First Name (optional)

  • Default Task Assignee Last Name (optional)

  • Default Task Assignee Email (optional)
